Enrollment & Registration Information
Welcome to the Crawford County School District! We are excited to partner with you and your family.
All students new to Crawford County Schools, as well as students returning from another school district or home school, must complete the registration process. If your child has received Gifted services, ESOL services, or has an IEP, please bring any available documentation at the time of registration.
You must bring the appropriate documents with you to complete the registration process for verification.
Required Documents:
· Copy of the student’s Birth Certificate
· Georgia Immunization Form 3231 (Out-of-state students must contact the Crawford County Health Department at 478-836-3167)
· Georgia Eye, Ear, and Dental Form 3300
· Student’s Social Security Card or signed waiver
· Photo Identification of parent or legal guardian
· Custody or Guardianship paperwork (if applicable)
· Withdrawal form from previous school (if applicable)
· Most recent report card (if applicable)
· Discipline history for middle and high school students
· Official transcript for all high school students
Proof of Residence (At least one required):
· If you rent: Current lease agreement, utility bill (no more than 30 days old), or utility connection receipt
· If you own: Mortgage statement (no more than 30 days old), property tax bill, utility bill (no more than 30 days old), or utility connection receipt
· If you live in a multi-family residence: Property Owner/Renter Affidavit Form (signed and notarized) plus residency documentation provided by the homeowner
